What is Transition Planning?
Transition planning is the process of preparing students—especially those with disabilities—for life after they leave the school system. This planning often begins as part of the Individualized Education Program (IEP) by age 14–16, but it’s never too early to start thinking ahead.

We help the student and family clarify post-school goals in education, employment, daily living skills, and community participation.
2. Creating the Transition Plan
We work with you to build a personalized plan that may include training opportunities, internships, skill-building activities, and community resources.
